Waiver

By entering my name below and submitting this form, I acknowledge, agree, and understand that there are certain risks and hazards involved in participating in adult flag football, including the potential for serious injury to myself and others. I further agree to the following:

• I voluntarily accept and assume all risks associated with playing flag football.
• I release and hold harmless Terrance Dyson/TFL, its designees, and any owners or lessees of fields used by my team from any and all liability for injury, disability, death, or loss or damage to person or property arising from my participation, regardless of cause.
• This release also includes any effects of exposure to COVID-19 or other illnesses contracted while participating in this program.
